I came across the book "How Not to Die" by Michael Greger. He creates a chart a lot like this in his book in the section about applying the ideas daily. He says that he originally had it on a white board in his kitchen.

I like that "exercise" is a checkbox. (He argues that the CDC recommended amount of exercise is very low and very modest. He wants 90 mins/day of brisk walking (4 MPH) or 40 mins brisk exercise. It seems that the Fitbit that I have does a decent job of recording "active minutes" for this. It counts certain activities/heart rates at a simple rate (one minute per minute of exercise) and doubles active minutes.
It makes me think I can adapt it to a daily dozen of other things that "should be done for heath" each day.... meditation, stretching, making something. I have tried other versions of these before.
